139s into flight, a cloud of white vapor was seen around the second stage of Falcon 9. Several seconds later, the rocket exploded. The failure was attributed to a strut which was holding a COPV in place failing below its rated load causing it to come loose and strike the second stage liquid oxygen tank. The Dragon spacecraft survived the initial explosion however was destroyed on impact with the ocean as the parachutes had not deployed. This was because the flight software has not accounted for a failure in this manor. First use of OCISLY however no landing attempt was made as a result of the failure
