Details:
First crewed flight from SLC-40. This mission flew with a reduced crew of 2 instead of the usual 4 as the crew of Starliner CFT would return on Crew Dragon after issues experienced with the Boeing Starliner spacecraft. After successful deployment of Crew Dragon, the second stage deorbit burn did not go to plan. While the second stage was disposed of safely in the ocean, it fell short of the targeted area. This prompted an investigation by the FAA requiring SpaceX to stand down from Falcon launches however SpaceX did initially voluntarily pause launches to investigate the root cause of the issue. This was the third FAA investigation as a result of a Falcon 9 anomaly in under three months
